Pharmacist Referral Forms

The use of referral forms enhances the service pharmacists provide to consumers, and minimises the possibility of inaccurate or incomplete information being relayed back to the General Practitioner. The forms also add continuity of care for those who need to consult more than one health care professional, and instill confidence in the consumer that they are receiving advice from the appropriate health care professional.

Pharmacist Referral Form pads are now available from Fildes. Contact your Fildes State office for further information.
