6CPA MedsCheck and Diabetes MedsCheck Program

From 1 February 2018, a number of changes were made to the 6CPA medication adherence and medication management programs. This module outlines the delivery of the 6CPA MedsCheck and Diabetes MedsCheck Program, including the new rules and requirements.

Information in this module should be used in conjunction with 6CPA program rules.

This activity has been accredited for 0.5 hours of Group 1 CPD (or 0.5 CPD credits) suitable for inclusion in an individual pharmacist’s CPD plan, which can be converted to 0.5 hours of Group 2 CPD (or 1 CPD credit) upon successful completion of relevant assessment activities.

  • Learning objectives
    After completing this activity, pharmacists should be able to:
    • Identify new 6CPA MedsCheck and Diabetes MedsCheck program requirements
    • Discuss the rationale for health outcome data collection
    • Complete MedsCheck and Diabetes MedsCheck services in accordance with 6CPA program rules.

    Competency Standards (2016) addressed: 1.1.1, 1.1.2, 1.3.1, 1.3.4, 1.5.1, 2.1.3, 2.3.1, 2.4.3, 3.2.2. 4.3.3, 4.5.3.
  • 2010 Competencies
    Competency Standards (2010) addressed: 1.2.1, 1.2.2, 1.3.1, 2.3.3, 2.6.3, 3.1.1, 3.3.3, 6.2.2, 6.2.4, 6.2.5.
