Becoming accredited to conduct medication reviews is an exciting professional career pathway for pharmacists. Accredited pharmacists form an integral part of the healthcare team, taking a lead role in ensuring practical, quality medication management for consumers in their homes or aged care facilities. This online module satisfies the Stage 1 training requirements for accreditation with the Australian Association of Consultant Pharmacy (AACP).

Participants will need to complete pre-reading before proceeding to the 10 online modules, eight of which have associated assessments. The modules will provide the necessary theory needed to conduct medication management reviews and will work through a sample HMR and RMMR to demonstrate its application.

Learning objectives

After completing this module, pharmacists should be able to:

  • Discuss principles for effective communication with patients, carers, general practitioners and other health professionals
  • Discuss principles for achievement of effective collaborative working relationships with general practitioners, community pharmacists and other members of the healthcare team
  • Demonstrate an in-depth knowledge of the comprehensive medication review process, and knowledge of relevant and appropriate tools
  • Describe how to take a medication history
  • Describe principles of motivating and facilitating behavioural change
  • Explain strategies to improve patient adherence to medication regimens and lifestyle changes
  • Demonstrate how to counsel patients to promote self-management of medicines (where appropriate)
  • Discuss principles of achieving skills to identify, resolve and prevent actual or potential medication-related problems.
